Nuform Health review
Confirm the complete quote
Nuform publishes multi-month weight-loss blend packages and describes a dedicated care team. Confirm the prescription ingredients and separate care charges before comparing its monthly headlines.
Our verdict
Nuform makes package totals visible, but its pages do not settle one complete treatment price. Get the exact prescription, supplied quantity and care fees together, particularly where different pages describe different starting figures or physician charges.

What it is
Nuform is a telehealth service offering weight management alongside hormone and other care. Its terms identify Nuform Health and Wellness, LLC, and describe independent clinicians working through Christopher Elia, D.O. P.C., doing business as Nuform Health and Wellness, P.C. The platform and the clinician have different roles; a public offer does not guarantee a prescription.
How it works
Nuform describes an assigned provider and specialist, direct phone or text contact and four-week follow-ups. Its terms allow live and asynchronous consultations and limit access to locations where services are available. The care page says GLP-1 weight-loss treatment does not require bloodwork, while other pages discuss lab-led care more broadly. Confirm what your selected treatment requires and which clinician and pharmacy will handle it.
Pricing transparency
The general weight-loss page lists a GLP starter package at $389 for a three-month label and maintenance at $589 for five months. GLP+GIP packages are $499 for 2.5 months, $780 for 4.5 months and $980 for 6.5 months. The GLP starter describes three billing increments, while a duplicated section says three-month increments. GLP maintenance describes five-month billing increments; the GLP+GIP packages describe increments of 2.5, 4.5 and 6.5 months, respectively. These labels do not settle exact payment dates, a monthly invoice or the quantity dispensed. A separate women’s page starts GLP at $149 a month and excludes consultation. The care page lists a $170 annual physician fee; the homepage calls that fee one-time. The refund policy separately lists a $50 consultation charge, without establishing whether it is credited against the physician fee. Another care page says there are no membership fees but excludes consultation and labs. Ask which charges apply to your quote, including delivery. Medication orders become nonrefundable when placed; consultation and physician-fee refunds have separate conditions.
Branded vs compounded
Nuform markets compounded GLP and GLP+GIP vitamin blends, but the kit names do not establish the exact active ingredients or preparation. Its FAQ separately says it offers oral and injectable semaglutide; that does not identify either priced blend. Nuform’s disclaimer says compounded medications are not FDA-approved. Confirm the prescription name, route, ingredients, concentration and supplied quantity. The reviewed public pages, including the terms and privacy policy, do not identify a dispensing pharmacy by name.

How we assessed this. This review reflects GLP Loss’s own read of Nuform Health’s publicly-stated model — pricing transparency, clinical oversight, medication clarity, and access — against our editorial methodology. We publish no fabricated ratings or first-hand outcomes. A partnership never changes the assessment — see how we make money.
Frequently asked
- Is the advertised monthly figure collected each month?
- The page describes three billing increments for the $389 GLP starter and five-month increments for its $589 maintenance package. GLP+GIP packages describe increments of 2.5, 4.5 or 6.5 months. A duplicated GLP section instead says three-month increments. Confirm the charge due now, later payments and shipment schedule; these labels do not establish exact dates, a monthly invoice or supplied quantity.
- Does GLP+GIP vitamin blend mean a particular tirzepatide prescription?
- The blend name alone does not establish that. Ask the clinician for the actual medicine and ingredients. The separate semaglutide FAQ also does not bind a particular preparation to either blend price.
- Are consultation and physician fees included?
- The women’s page excludes consultation. The refund policy lists $50 for consultation, while other pages describe a $170 physician fee with different timing language. Confirm which applies and whether one is credited against another before paying.
- Does the plan renew automatically?
- The care pages say plans do not automatically renew and describe reordering another kit. The general terms also allow automatic payments if you enroll in them. Confirm the payment authorization for your selected plan and the timing of any next charge.
- What can be refunded if I change my mind?
- The policy allows a consultation refund if you cancel at least 24 hours before the appointment or are declined for treatment. Completed consultations are nonrefundable. Physician fees have their own conditions, and medication orders are final once placed with the pharmacy.
- Will I need to pay for bloodwork?
- The care page exempts GLP-1 weight-loss therapy from its bloodwork requirement and lists optional testing from $95. Other pages describe lab-based assessment more generally. Confirm whether testing is required for your selected treatment and what it would cost.
