Fifty 410 review

Two ways to buy

The low tirzepatide figure belongs to a three-month program, and the pay-as-you-go rate is a separate, higher number.

Our verdict

Fifty 410 runs a Monthly and a 3 Months toggle, and the two tabs are not the same offer at two lengths. On the three-month tab, tirzepatide is sold in dose tiers: $399, $499 and $549 in total, printed as $133, $166 and $183 a month. On the monthly tab both molecules collapse to $199, described as pay as you go with all dosages included. So the entry figure requires $399 up front and covers only the starting dose tier, while the pay-as-you-go buyer at a high dose pays $199 rather than $183. The site is unusually plain about which figure attaches to which term, and that is to its credit.

What it is

A telehealth platform listing compounded semaglutide and tirzepatide by weekly injection, with hair-care and wellness products marked as coming soon.

How it works

A free online intake reviewed by a licensed provider, with no card required to start and payment requested only if a prescription is issued. The platform states that it does not compound, manufacture or distribute anything, and that independent pharmacies prepare what is prescribed.

Pricing transparency

Tirzepatide on the three-month program: a starter tier at $399 in total, a mid-dose tier at $499 and a high-dose tier at $549. The cards display those as $133, $166 and $183 a month, each total divided by three and rounded down. The pay-as-you-go option is $199 a month for a one-month supply, offered for compounded semaglutide and compounded tirzepatide alike, with all dosages included. Semaglutide has no three-month program, so $199 is its only published rate. The FAQ states plainly that no membership is required and that there is no membership fee.

Branded vs compounded

Compounded semaglutide and compounded tirzepatide, both subcutaneous injections. Compounded drugs are not FDA-approved and are not reviewed by the FDA for safety, efficacy or quality before dispensing, and the site says so in those terms on its program page. The boxed warning for thyroid C-cell tumors is reproduced there too. No milligram strengths appear anywhere, so the dose tiers are named rather than quantified.

FDA warning letter on record

The FDA issued a warning letter naming Aspen Aesthetics dba Fifty 410, the company behind fifty410.com — the site this review covers.

We publish this because it is a matter of public record and a reader weighing this provider should have it. A warning letter is the FDA setting out alleged violations; it is not a court finding, and a company may have responded or corrected since. It does not remove Fifty 410 from our rankings, and it does not change our verdict — read the letter and decide for yourself.

Frequently asked

Is Fifty 410 tirzepatide $133 a month?
On the three-month program, yes, and the program page says so directly. It is $399 in total for the starting dose tier, which divides into $133. Month to month the same molecule is $199, and that tab is labeled pay as you go.
What does semaglutide cost here?
$199 a month, listed as a one-month supply on the pay-as-you-go option. The GLP-1 program page describes compounded semaglutide as listed from $199 per month. There is no three-month semaglutide program, so no lower figure exists for it.
Does the price rise with the dose?
On the three-month program it does, across tiers at $399, $499 and $549. On the pay-as-you-go option it does not, because that card states all dosages are included at $199. A buyer expecting to titrate high should compare those two structures carefully.
Is there a membership fee?
No. The FAQ states that no membership is required and that programs are purchased directly, and a separate answer says there is no membership fee. Free shipping in refrigerated packaging is included in the program price.
Is Fifty 410 legit?
FDA has a warning letter on record against Aspen Aesthetics dba Fifty 410, the company behind fifty410.com, issued February 20, 2026 as letter 721481; it is linked further down this page. On this desk a warning letter discloses, it does not disqualify — it is a public record to read before you decide. Fifty 410 is a compounded tirzepatide sellers service. It displays a LegitScript certification claim, which we record as a claim and not as a verified fact: LegitScript's public checker is gated and returns the same page for any domain put into it, so no one can reproduce a check from it. What our record does not establish: which states it serves, whether it bills insurance and how its intake works. Ask for those in writing before you pay.
Is Fifty 410 expensive compared with other compounded tirzepatide sellers providers?
Fifty 410 publishes a starting figure of $199/mo. Across the 332 compounded tirzepatide sellers providers we review that publish a single monthly figure, those run $76 to $449 with a median of $179, which puts Fifty 410 above the median. The other 23 compounded tirzepatide sellers providers we review quote only after an intake or price by dose, so they are not in that comparison. A starting figure is not a bill — labs, consultations and shipping are often charged separately, so confirm what your own quote includes.
